What kind of personal information do we have and where did we get it?
Much of the personal information that we maintain comes directly from you. You provided much of this information to us on your application or request for insurance or other products we offer. We may contact you by telephone or mail for additional information. We also keep information about the types of products and services you purchase from us, as well as account balances and payment history and information we receive from medical records or medical professionals. We collect nonpublic personal information from these additional sources: consumer reporting agencies, government agencies and our affiliates.

Depending on the nature of the transaction you are completing with us, you may be required to provide The Unity Group, our affiliates, or other entities working on our behalf with information. That information may include, for example, your name, address, birth date, phone number, health information, e-mail address, the types and numbers of the policies you hold, Social Security number, credit card information, driver's license number, accident/violation history, information about vehicle operators, mortgages, lien/lease holders, or vehicle information.

Information we may disclose to third parties:
We may disclose all of the information we collect, as described above, about our customers or former customers, to companies that perform marketing services on our behalf, third party service providers or to other financial institutions with whom we have joint marketing agreements. We will not disclose information about our customers or former customers outside the parameters permitted by law.

Non-affiliated third parties to whom disclosures may be made:
The Unity Group does not disclose any of your personal information to companies or organizations not affiliated with us that would use the information we have provided them to contact you about their own products and services. We may use your personal information to communicate with you about products, features and options in which you have expressed an interest, or that we believe may be of interest to you.

In addition, we will not disclose information about our customers or former customers outside the parameters permitted by law.

How do we protect your personal information?
We restrict access to nonpublic personal information about you to those employees who need to know that information in order to provide products or services to you. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your nonpublic personal information.

Should your relationship with The Unity Group end, your personal information will remain protected in accordance with our privacy practices.

Reservation of the right to disclose information in unforeseen circumstances:
In connection with the potential sale or transfer of its interests, The Unity Group and its affiliates reserve the right to sell or transfer your information (including but not limited to your address, name, age, gender, zip code, state and country of residency) to a third party entity that (1) concentrates its business in a similar practice or service; (2) agrees to be The Unity Group's successor in interest with regard to the maintenance and protection of the information collected; and (3) agrees to the obligations of this privacy statement.

Our policy regarding dispute resolution:
Any controversy or claim arising out of or relating to our privacy policy, or the breach thereof, shall be settled by arbitration in accordance with the rules of the American Arbitration Association.
